    Please assist us by providing the following information at the time of your visit:

    • Any recent X-rays from a previous dental provider (please have them forwarded to our office in advance)

    • A current list of all medications you are taking

    • Your dental insurance information so we can help facilitate claim submission

    Receiving this information ahead of time allows our team to review your records thoroughly and ensure a smooth, efficient visit.

As a courtesy, we submit claims directly to your insurance provider and provide benefit estimates prior to treatment whenever possible.   • If you experience a dental emergency after office hours, please call our main office number. Our voicemail system will provide instructions for reaching the doctor through the designated emergency number.
